Choosing keywords outside of their topical authority is basically the one, big mistake
Basing their decisions off of search volumes.
Not performing volume and competition analysis to identify best keywords with reasonable chance of ranking for.
Just thinking up the keywords they want to rank for rather than doing keyword research to see how/what people are actually searching.

You mean like one of my recent local clients who wanted to rank for “therapist.” That’s it, just “therapist.”
Trying to compete with big brands. Start with lower competition and build yourself from there.
Assuming they are trying and getting the basics right, a few common mistakes I see are:
1. Not grouping by intent or understanding what the intent is for a keyword. If someone is searching for “San Francisco personal injury lawyer,” they don’t want a page talking about 20 different cities in California. It should be San Francisco-specific. If someone is searching for “How long do personal injury claims take in California,” they want a page that answers that question, not just an FAQ in a dropdown on a San Francisco personal injury lawyer page. By having that be a separate page, it will be more likely to rank, and it can link back to the other page. We like to create whole clusters around each practice area topic like that, all linking back to the main page and maybe to each other. If it is just a PAA, then you might just create an FAQ.
2. Not checking local search volume in Keyword Planner. Tools like SEMrush and Ahrefs show global volume and by country, but not by city or region. For my clients (Law Firms), we need to know how many people are searching for something like “back injury lawyer near me” in a given city or region. It also can help identify keywords to target as people in your target area are searching for them and sometimes they aren’t things I wouldn’t have thought of, like in the North East, a lot of people search about dog runs instead of dog parks. In certain areas, you’ll find volume for things like counselor or solicitor.
3. Not checking the SERP. You may think you know the intent, but check the SERP to see what Google wants to show as the results. If it’s all blog posts and you are writing an FAQ, maybe you need to write a blog instead. For a law firm, if the results are all medical sites, or government sites, or universities, or high authority sites or news sites, maybe you need to rethink whether you can rank.
